Current vehicle registration (vehicle registration must be in your name or you must provide a lease agreement specifically covering that vehicle

Five years work history, complete with names and addresses

Proof of bobtail or deadhead insurance with liability limits of at least $500,000 for vans and straight trucks and $2,000,000 for tractor trailers

Valid CDL for the size unit you intend to drive (Must have H endorsement)

Copy of current (less than six months old) long form D.O.T. physical

Social security card or W-9 form

Nationwide cellular phone
